The incidence of pediatric inflammatory bowel disease is increasing, and the risk of surgery is remarkable. Pediatric Severe Ulcerative Colitis could require surgical treatment, three- or two-stages are recommended and laparoscopy, when feasible, should be the preferred approach. Surgery is a valid alternative in case of localized Crohn Disease in children with prolonged postoperative remission.
