We describe the case of an 11 year old obese child with chronic elevation of aminotransferases and enlarged liver with severe steatosis. Nonalcoholic steatohepatitis was diagnosed; although it is frequently found in patients with obesity, it can also be caused by other etiologies, which must therefore always be ruled out in the obese child.
