We report the case of a vegetarian 3 year-old boy who presented anasarca, diarrhea, vomiting, abdominal pain and weakness for ten days. Laboratory and instrumental studies revealed hypoalbuminemia and pleural effusion. At first we supported the diagnosis of malnutrition but the persistence of symptoms directed our diagnosis to Menetrier’s desease. Endoscopy showed erythematosus gastric mucosa of body and fundus and biopsies revealed evidence of CMV infection.
