Chronic intractable constipation is a challenging clinical entity defined as failed response to optimal medical treatment for at least three months. It is an uncommon condition related to primary or secondary neuromuscular disorders of the colon, with high burden for young patients and their families. It requires motility investigations (radiomarker study, ano-rectal and colonic manometry) in tertiary/quaternary care facilities. A tight cooperation with surgical teams has a pivotal role in delivering the best care strategy for each patient.
