GER has long been considered a major cause of ALTE but its relation is still controversial. The timing and role of investigation for GER need to be clarified. We describe a 45-days-old infant presenting with apnea, hypertonia, erythrosis and failure to thrive. In this patient a 24 hours esophageal pH-impedance monitoring combined with cardiorespiratory monitoring was essential for diagnosis and management.
